THIS IS A LEGALLY BINDING CONTRACT BETWEEN YOU AND HAUS LLC.

In consideration for the opportunity (and/or the opportunity for my child) to participate in HAUS 1-on-1 Mentorship (“HAUS”) taking place virtually running from April 1, 2025 – August 31, 2025 (the “Program”), I do hereby release, hold harmless and will not, individually, collectively or as parent(s) or legal guardian(s) of the participant, sue or bring any legal action or proceeding against HAUS and any or all of its directors, officers, employees, instructors, volunteers, staff, affiliates, related entities and/or successors and assigns (collectively, the “Releasees”) for or on account of any injury, pain, suffering, loss or damage of any kind that the Participant may sustain by virtue or arising out of, directly or indirectly, Participant’s participation in the Program. Me, or my child as the case may be, are referred to herein as “Participant,” and individually or collective with my parent or legal guardian, the “Releasor(s).” This Release and Waiver may be pleaded as a complete defense to any action or other proceeding which may be brought, instituted, or taken by any of the Releasors, the Releasors’ guardians, heirs or legal representatives against any of the Releasees.

The Releasors acknowledge that Participant’s attendance in the Program is voluntary. The Releasor(s) further acknowledge and agree that (i) such participation is without assumption of responsibility or risk of any kind by any of the Releasees, (ii) the Releasees do not make any warranties of any kind with respect to the Program or its safety, and (iii) the Releasees are not medical doctors nor is any of the information disseminated during the Program is meant to be or shall be construed as medical and/or mental health advise or diagnoses.

The Releasors irrevocably consent without further consideration to allow the Releasees to use, publish and copyright Participant’s name, nickname, voice, picture, portrait, likeness, characteristics, live or recorded performance, photograph, signature or facsimile thereof, biographical materials, all intellectual property, and all other materials relating to or identifying the Participant and any and all materials resulting from or derived from Participant’s participation in the Program in all forms of media and promotion now known or hereafter created, including without limitation, video, and social media (e.g. Instagram, Twitter, etc.).

The Releasor(s) hereby release, waive and forever discharge the Releasees from and against any and all liabilities, claims, demands, and causes of actions whatsoever, at law or in equity, to the fullest extent permissible by law, directly or indirectly arising out of or related to any loss, damages, injury or death (including but not limited to damages or losses caused by the negligence, fault or conduct of any kind on the part of the Releasees) that may be sustained by Participant or Participant’s family members arising out of, directly or indirectly, Participant’s participation in the Program.

This Release and Waiver is not in satisfaction of any damages sustained nor as compensation for injuries, nor in settlement of any claim nor damages. The opportunity of the Participant to attend and participate in the Program is offered as consideration for this Release and Waiver and the agreements contained herein, and the Releasors hereby acknowledge the adequacy and receipt of such consideration. It is understood and agreed that the acceptance of this Release and Waiver by the Releasees is not an admission of any liability on their part and that each of the Releasees expressly denies any liability with respect to the Program.

This Release and Waiver is irrevocable. Accordingly, no breach by the Releasees of any of their respective obligations and/or duties owed to the Releasor(s)(whether or not arising under this Release and Waiver) shall entitle the Releasor(s) to terminate or rescind this Release and Waiver, and the Releasor(s) hereby waive the right, in the Program of any such breach, to equitable relief or to enjoin, restrain or interfere with any of the Releasees, it being understood that the Releasor(s) shall have no right whatsoever to attempt to recover any damages with respect to any such breach.

The Releasor(s) has/have carefully read this Release and Waiver and understands the contents of it.
