HAUS WAIVER & RELEASE OF LIABILITY

PLEASE READ THIS DOCUMENT CAREFULLY. THIS IS A LEGALLY BINDING AGREEMENT.

In consideration for the opportunity to participate (and/or for my child to participate) in coaching,

training, events, sessions, and programs provided by HAUS LLC (“HAUS”), including but not limited to

one-on-one coaching and group-based programs such as HAUS Circle (collectively, the “Program”), I

agree to the following:

1. Voluntary Participation

I understand that participation in the Program is voluntary and may be discontinued at any time. I

acknowledge that I (or my child) am choosing to participate at my own discretion.

2. Scope of Services

I understand that HAUS provides coaching, personal development, and educational services focused

on mindset, performance, and self-awareness. I acknowledge that HAUS does not provide medical

care, mental health treatment, psychotherapy, or diagnosis of any kind. These services are not a

substitute for licensed medical or mental health care. I understand that I am responsible for seeking

appropriate professional care as needed.

3. Assumption of Risk

I understand that participation in coaching sessions and related activities may involve inherent risks,

including physical, emotional, and psychological responses. I voluntarily assume all risks associated

with participation in the Program, whether known or unknown, and accept full responsibility for my (or

my child’s) participation.

4. Breathwork, Meditation, and Physical Practices

I understand that sessions may include breathwork, meditation, visualization, light movement, or other

mind-body practices. I acknowledge that these practices may produce physical, emotional, or

psychological effects, and that participation is voluntary. I agree to monitor my own (or my child’s)

condition and to modify or stop participation at any time if discomfort arises. I understand that certain

practices may not be appropriate for individuals with certain medical or mental health conditions

(including but not limited to respiratory, cardiovascular, neurological, or psychological conditions), and I

accept full responsibility for participation.

5. Release of Liability

To the fullest extent permitted by law, I hereby release, waive, and discharge HAUS LLC and its

owners, officers, employees, contractors, agents, and affiliates from any and all claims, liabilities,

damages, or causes of action arising out of or related to participation in the Program.

6. Recording and Media Release

I understand and agree that sessions may be recorded (audio and/or video) by HAUS for coaching,

educational, and business purposes. I grant HAUS permission to use my (or my child’s) image,

likeness, voice, and participation in recordings, photographs, video, or other media in any format for

lawful business purposes, including content creation and marketing. HAUS will use reasonable

discretion in the use of such materials and will not intentionally share sensitive or personal information.

HAUS will make a good-faith effort to obtain additional permission before using identifiable content(such as a participant’s name or clear image) in public-facing materials. I understand that participation

in group settings may result in incidental or non-identifiable inclusion in content without additional

consent. I waive any right to inspect or approve the final use of such materials and release HAUS from

any liability related to their use.

7. Data and Information Storage

I understand that participant information, session content, and recordings may be stored securely by

HAUS for internal use, program delivery, and related business purposes.

8. Applicability

This agreement applies to all participation in HAUS programs, services, and sessions, whether

conducted virtually or in person, and remains in effect for as long as I (or my child) participate in any

HAUS offering.

9. Governing Law

This Agreement shall be governed by and construed in accordance with the laws of the State of

Connecticut.

10. Indemnification

I agree to indemnify and hold harmless HAUS LLC and its owners, officers, employees, contractors,

agents, and affiliates from any claims, damages, liabilities, costs, or expenses (including reasonable

attorneys’ fees) arising out of or related to my (or my child’s) participation in the Program or any breach

of this Agreement.

11. Severability

If any provision of this Agreement is found to be invalid or unenforceable, the remaining provisions shall

continue in full force and effect.

12. Acknowledgment and Agreement

I acknowledge that I have read and understand this Agreement, and that I am voluntarily agreeing to its

terms.

Parental Consent (if applicable)

I am the parent or legal guardian of the participant and have the authority to enter into this agreement

on their behalf.
